The form is used as a check and balance to make sure that we have all of the content and that we place it where it should be in the document.
Corrections Log Form (pdf 23k)
Use this form to list corrections that need to be made to your document.
The form includes spaces for page number and description of the corrections. We also have check boxes to ensure that the corrections are made to create a dynamic document.
